The purpose of this questionnaire is to help you determine whether or not my services can benefit you.  If you find yourself answering "yes" to any 3 of the following questions; my knowledge and expertise can help.
 
1. Is there a legal mandate or requirement?
2. Do you or the individual have permanent limitations as a result of an injury or incident?
3. Will the permanent limitations effect you or another individuals employability or life-style?
4. Can you or another individual resume their past employment?
5. Can the you or another individual resume any job which has been performed within the last fifteen years?
6. Do you or another individual have transferrable skills from a past employment experience which can be utilized in another job?
7. Will there be permanent limitations cause them to earn less?
8. Will training enable you or another individual the ability to return to employment with permanent limitations? 
9. At the completion of retraining could you or another individual earn as much or more?
10. Will your age, work experience, education and permanent limitations permit you to return to employment?
